1. Type of Performance (ex. Dance, Singing, etc.)

2. Primary Contact Name, Phone & Email

3. Group Performance Name

4. Participants’ Names

5. Performance Duration (ex. 1 min, 30 secs)

6. Participants Age Group range, if kids (ex. 2 to 5 years)-

 

Remember, this event is free, and everyone is welcome to participate in the cultural programs or simply attend. The TABALA event coordinators will reach out to registered participants for any additional information after you submit your registration.
